About the Role We are seeking a skilled and compassionate Nurse Practitioner to perform required Face‑to‑Face (F2F) encounters for hospice patients approaching benefit period recertification.

Key Responsibilities

  • Conduct Face‑to‑Face encounters for patients entering their 3rd benefit period and beyond.
  • Assess and document terminal prognosis, disease progression, functional decline, and symptom burden.
  • Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions.
  • Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
  • Maintain timely, compliant documentation aligned with Medicare and state regulatory standards.
  • Communicate findings effectively to physicians and clinical leadership.
  • Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.

Qualifications

  • Current and unrestricted Nurse Practitioner license in New Mexico.
  • National board certification as NP (FNP, AGNP, or similar).
  • Minimum 1-year clinical experience; hospice, palliative care, or geriatric experience strongly preferred.
  • Solid understanding of hospice eligibility criteria and disease trajectories.
  • Strong communication, interpersonal, and clinical assessment skills.
  • Reliable transportation and willingness to travel across assigned territory.
