Position Details
- Position: LVN Case Manager
- Schedule: Full-Time
- Annual Salary: $114,400
- Location: Medical Hill Healthcare Center
Key Responsibilities
- Coordinate and oversee resident care plans from admission through discharge.
- Collaborate with physicians, nursing staff, therapists, residents, and families to ensure continuity of care.
- Monitor clinical progress and identify opportunities to improve patient outcomes.
- Facilitate care conferences and interdisciplinary team meetings.
- Assist with discharge planning and transitions of care.
- Ensure compliance with federal, state, and facility regulations.
- Maintain accurate clinical documentation and case management records.
- Advocate for residents while promoting quality care and customer satisfaction.
Qualifications
- Current California LVN license in good standing.
- Case management, skilled nursing, or long-term care experience preferred.
- Strong assessment, communication, and organizational skills.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
- Knowledge of Medicare, Medi-Cal, managed care, and discharge planning processes is a plus.
