Patient Appointment Aide

Sonoma Post Acute is looking for a compassionate, dependable, and patient-focused individual to join our team as a Patient Appointment Aide!

In this important role, you will accompany residents to medical appointments and help ensure they feel safe, comfortable, and supported throughout the process. This is a great opportunity for someone who enjoys working with seniors and making a positive impact every day.

Responsibilities:

  • Accompany residents to outside medical appointments
  • Provide companionship and support during transport and appointments
  • Communicate professionally with healthcare providers and facility staff
  • Assist with appointment paperwork and basic coordination
  • Help ensure residents arrive safely and on time
  • Maintain resident dignity, comfort, and confidentiality at all times

Qualifications:

  • No certification or licensure required
  • Compassionate and patient demeanor
  • Reliable transportation record and punctuality
  • Strong communication and interpersonal skills
  • Ability to remain calm, organized, and professional
  • Experience working with seniors or in healthcare is a plus, but not required
