Job DetailsJob Location: Community & Danbury Hospital - CTPosition Type: Full TimeEducation Level: 4 Year DegreeJob Category: Health CareRegional Hospice is looking for an experienced Admissions RN to work with patients in the Community and at Danbury Hospital. We offer a generous benefits package including:  Medical, Dental, Vision, Life, 401K Match, Vacation/Sick Time, and Company Car!   The Admissions RN position is full-time, 40 hours/week rotating every other weekend.  The Admissions RN is responsible for the initial coordination and management of all patient care and other services for patients and their families.   Duties: Admits patients to service for Hospice Does evaluation for perspective hospice patients as needed Makes visits to patients as needed Provides skilled and supportive nursing care to patients and families. Performs assessment of patient and family, through observation, interviews, and analysis of records. Analyzes physical, emotional psychosocial, environmental and spiritual needs. Identifies past, present, and potential problems Begins development of a comprehensive care plan Prepares plan within agency time frame Completes paperwork required by agency, government and third-party systems. At admission, identifies patient and family needs for other home health care services. Communicates the patient’s plan of care to attending physician and other staff members. Qualifications Graduation from an accredited school of nursing; BSN preferred Currently licensed to practice as a Registered Nurse in the State of Connecticut. A minimum of two years of nursing experience, preferably in medical/surgical nursing required Hospice / End of life / Home care experience preferred Active client contact within past three years required. Certified Hospice and Palliative Care Nurse (CHPN) preferred Valid, clean driver’s license Required   #CB
