WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking an experienced Cardiovascular Advanced Practice Provider to meet the growing demand for Cardiology at Indiana Heart Physicians in Indianapolis, IN. This interested candidate will join a multi-specialty team of cardiologists, cardiac hospitalists, and advanced practice providers. 

                           

WHAT WE NEED:

Ideal candidate will be an experienced Cardiovascular Advanced Practice Provider, who will operate in an office/hospital setting and be responsible for caring for patient’s age 18 years and older who have cardiovascular conditions. The primary focus of the Advanced Practice Provider is seeing patients for regular follow-up visits in the ambulatory setting, as well as seeing patients in the acute setting.  Candidate will also have desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

  • Formulate medical diagnosis based on patient admitting status, and physical findings. Implement plan of care.

  • Communicate to Physicians, their assessment through surveillance exams, injuries, treatment and procedures as deemed necessary.

  • Develop individualized plan of care for all assigned patients. Consistently evaluate the effectiveness of care provided to the patient.

  • Perform interventions to meet holistic needs of patient, under the direction of the Medical Director, and under the leadership of the Client Resource Manager.

  • No weekend or holiday call

  • May travel to outreach sites

  • Common Diagnoses:

    • Atrial Fibrillation

    • Hypertension

    • Coronary Artery Disease

    • Heart Failure

    • Dyslipidemia

  • Daily Duties:

    • Complete patient health assessments

    • Diagnosis and treat individuals with current or potential cardiac needs

    • Offer lifestyle, medical and dietary suggestions

    • Develop treatment plan for patients

Qualifications

  • Physician Assistant Certified (PA-C) – Required - or -

  • Nurse Practitioner (NP) or Nationally Recognized Certification - National Certification Corporation (NCC)

  • Required Master's Degree Nursing / Patient Care

  • Preferred Doctorate Nursing / Patient Care

  • Basic Life Support Program (BLS) - American Heart Association

  • Advanced Cardiovascular Life Support (ACLS) - American Heart Association
