This is a remote position.


Critical Functions

1. Program Management & Operations

路聽聽聽聽聽聽聽 Oversee day-to-day execution of programs, including intake, eligibility, service delivery, and follow-up

路聽聽聽聽聽聽聽 Refine and maintain program policies, procedures, and workflows; identify operational risks and implement solutions to improve efficiency and participant experience

路聽聽聽聽聽聽聽 Assess and make any necessary improvements to intake process -- including evaluating, selecting, and implementing a long-term technology platform to replace current manual and stopgap workflows

路聽聽聽聽聽聽聽 Develop and maintain documented SOPs for all core program functions, written for transferability

路聽聽聽聽聽聽聽 Identify gaps in access or awareness and communicate insights to leadership

路聽聽聽聽聽聽聽 Support the development and management of program reports and budgets for leadership and funders

路聽聽聽聽聽聽聽 Track, analyze, and report on program outputs and outcomes

2. Community Partner Program (CPP) Administration

路聽聽聽聽聽聽聽 Transition full CPP ownership from the Director of Operations upon hire, including knowledge transfer and process documentation聽

路聽聽聽聽聽聽聽 Own the full CPP cycle from end to end -- annual grant funding outreach, partner agreement renewals, report collection and tracking, and payment processing

路聽聽聽聽聽聽聽 Serve as the primary point of contact for all Community Partners

路聽聽聽聽聽聽聽 Manage partner relationships proactively, ensuring agreements are current, reports are received on time, and partners have what they need to participate effectively

3. Community Outreach & Partnerships

路聽聽聽聽聽聽聽 Build and maintain a targeted referral network of healthcare providers, imaging centers, social service agencies, and community organizations serving underserved populations

路聽聽聽聽聽聽聽 Lead outreach meetings and presentations with referral partners

路聽聽聽聽聽聽聽 Support outreach efforts to increase awareness and utilization of resources

路聽聽聽聽聽聽聽 Serve as a point of contact for complex or high-priority referrals

4. Grants & Reporting

路聽聽聽聽聽聽聽 Support grant applications by providing program data, narratives, and insights

路聽聽聽聽聽聽聽 Track and report program outcomes for funders and internal use

路聽聽聽聽聽聽聽 Maintain accurate and organized program documentation

5. Team & Cross-Functional Collaboration

路聽聽聽聽聽聽聽 Supervise the Program Coordinator; provide day-to-day guidance, escalation support, quality oversight, and professional development

路聽聽聽聽聽聽聽 Partner with the Director of Operations on administrative and operational needs

路聽聽聽聽聽聽聽 Collaborate with Development on fundraising support including data, impact stories, and reporting

路聽聽聽聽聽聽聽 Collaborate with marketing partners to develop and maintain referral tools such as program one-pagers, eligibility summaries, and intake materials




Requirements

Qualifications

聽Required:

路聽聽聽聽聽聽聽 Bachelor's degree in public health, social work, nonprofit management, or a related field, or equivalent professional experience

路聽聽聽聽聽聽聽 Minimum of 5 years of experience in program coordination or management within nonprofit, healthcare, or human services聽on a national level

路聽聽聽聽聽聽聽 Experience with grant-making and grant-distribution program model

路聽聽聽聽聽聽聽 Minimum of 3 years of Supervisory experience

路聽聽聽聽聽聽聽 Experience in medical advocacy, patient navigation, or public health

路聽聽聽聽聽聽聽 Demonstrated ability to evaluate and implement technology or process improvements in a program setting

路聽聽聽聽聽聽聽 Strong organizational, analytical, and project management skills

路聽聽聽聽聽聽聽 Self-starter who thrives in a small, fast-paced environment

路聽聽聽聽聽聽聽 Excellent written and verbal communication skills

路聽聽聽聽聽聽聽 Comfort working in a remote team environment

路聽聽聽聽聽聽聽 Must work EST hours 9am-5pm

Preferred:

路聽聽聽聽聽聽聽 Spanish proficiency

路聽聽聽聽聽聽聽 PMP certification or equivalent project management credential

路聽聽聽聽聽聽聽 Familiarity with referral-based or patient/client service models

路聽聽聽聽聽聽聽 Experience with community outreach and referral network development

路聽聽聽聽聽聽聽 Familiarity with grant-funded programs and reporting requirements

Salary
$
70,000-$80,000 annually; exact compensation is based on a range of factors including the labor market, job level, internal equity, and budget. Offers will take into consideration the candidate's skills, education, experience, geographic location, and other necessary credentials.
