What You'll Do
- Coordinate and complete MDS assessments in accordance with state and federal regulations.
- Ensure accurate and timely submission of MDS documentation.
- Collaborate with interdisciplinary teams to develop and update resident care plans.
- Monitor clinical documentation to support reimbursement and care quality.
- Participate in quality assurance and compliance initiatives.
- Serve as a resource for nursing staff regarding MDS and care planning processes.
What We're Looking For
- Current RN or LPN license in good standing.
- Previous MDS experience in a skilled nursing or long-term care setting preferred.
- Strong knowledge of Medicare, Medicaid, and PPS processes.
- Excellent organizational and communication skills.
- Ability to work independently while collaborating effectively with team members.
- Commitment to providing exceptional resident-centered care.
