Gateway Post Acute is a premier skilled nursing and post-acute rehabilitation facility located in Porterville, CA. Known for our compassionate care, clean environment, and supportive team culture, we are currently seeking a PRN Occupational Therapist (OT) to join our rehabilitation team.

This is an excellent opportunity for a motivated therapist who enjoys flexibility while making a meaningful impact on patients’ functional independence and quality of life.

General Purpose

The Occupational Therapist (OT) evaluates and treats patients referred by physicians, develops and implements individualized treatment plans, collaborates with interdisciplinary team members, and maintains accurate and timely clinical documentation in accordance with federal, state, and facility regulations.

Essential Duties and Responsibilities

  • Evaluate patients and establish individualized occupational therapy treatment plans

  • Treat patients in accordance with physician orders and approved plans of care

  • Document evaluations, daily treatment notes, progress notes, recertifications, and discharge summaries per regulatory and payer requirements

  • Educate patients and families on adaptive techniques, safety, and functional independence

  • Collaborate with nursing, therapy, and interdisciplinary teams to support optimal patient outcomes

  • Participate in patient care conferences, rehab meetings, and discharge planning as needed

  • Recommend and facilitate ordering of adaptive equipment and durable medical equipment (DME)

  • Maintain safe, clean, and organized treatment areas

  • Report equipment issues to ensure proper operation and safety

  • Maintain patient confidentiality and uphold all resident rights and privacy standards

Qualifications

Education & Experience

  • Bachelor’s degree in Occupational Therapy required

  • Master’s or Doctorate degree preferred

  • Skilled Nursing Facility or post-acute experience preferred

Licensure & Certifications

  • Current, active California Occupational Therapist license in good standing

  • Current CPR/BLS certification

Skills

  • Strong clinical judgment and patient-centered approach

  • Proficient in electronic documentation systems (PointClickCare and/or Casamba preferred)

  • Excellent communication, time-management, and organizational skills

  • Ability to work independently and as part of a collaborative team
