Gold Coast Physical Therapy is seeking a dedicated and patient-focused Physical Therapist to join our Home Division.  This role provides outpatient-level physical therapy services in the comfort of patients' homes, allowing clinicians to deliver highly personalized care while helping patients improve mobility, function, independence and quality of life. 

We’re looking for a clinician who values collaboration, consistency, and delivering high-quality care in a supportive and well-established environment.

What You’ll Do

• Evaluate and treat patients in their homes using evidence-based physical therapy interventions

• Develop individualized treatment plans focused on functional outcomes and patient goals

• Educate patients and caregivers on exercises, safety, mobility, and injury prevention

• Document evaluations, progress notes, and discharge summaries accurately and timely

• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care

• Monitor patient progress and modify treatment plans as appropriate

• Maintain compliance with company policies, professional standards, and state regulations
