Basic Function:

The Practice Transformation Specialist (PTS) is responsible for engaging healthcare providers, community organizations, institutions of higher education, and other stakeholders in evidence-based initiatives designed to reduce substance use disorder (SUD)-related morbidity and mortality. The PTS serves as a facilitator, educator, and quality improvement consultant, supporting the implementation of opioid stewardship, harm reduction, recovery support, and evidence-based substance use prevention and treatment strategies. 


Success in this role requires the ability to build and maintain strong relationships with healthcare providers and community partners; assess organizational needs; deliver educational interventions and academic detailing; facilitate stakeholder collaboration; and support the adoption of evidence-based practices related to opioid prescribing, Prescription Monitoring Program (PMP) utilization, medications for opioid use disorder (MOUD), stigma reduction, overdose prevention, and care coordination. 


To address these challenges, the PTS may utilize: 


  • Academic detailing methodologies and evidence-based provider education strategies. 
  • Quality improvement methodologies, including workflow assessment, gap analysis, goal setting, implementation planning, and Plan-Do-Study-Act (PDSA) cycles. 
  • Evidence-based opioid prescribing guidelines, including the CDC Clinical Practice Guideline for Prescribing Opioids for Pain. 
  • Motivational Interviewing, patient-centered communication, and trauma-informed care principles. 
  • Harm reduction approaches, overdose prevention strategies, naloxone education, and referral pathways. 
  • PMP integration and utilization best practices. 
  • Team-based care models that support SUD prevention, identification, treatment, and recovery. 
  • Social Determinants of Health (SDOH) assessment and referral resources. 
  • Health equity, cultural competency, and stigma reduction approaches. 
  • Adult learning principles and educational program design. 
  • Data collection and evaluation methods to measure educational effectiveness and practice change. 

The PTS serves as a consultant and implementation partner, helping providers and organizations assess current practices, identify opportunities for improvement, and implement sustainable, evidence-based interventions that improve patient outcomes and strengthen community capacity to address substance use disorders. 


The PTS coordinates task-related efforts by creating, participating in, and facilitating partnerships with healthcare organizations, professional associations, academic institutions, community-based organizations, employers, and state agencies. Responsibilities include supporting stakeholder engagement activities, facilitating meetings and educational sessions, coordinating outreach efforts, sharing resources and best practices, reducing duplication of effort, and promoting alignment across initiatives. 


The PTS provides technical assistance and education to healthcare providers and community stakeholders, supports curriculum development and educational program implementation, assists with needs assessments and gap analyses, and contributes to project evaluation and reporting activities. This role requires excellent communication, facilitation, relationship-building, and project management skills, as well as the ability to independently manage multiple priorities in a dynamic public health environment.

Role and Responsibilities:

  • Recruit, engage, and maintain relationships with healthcare providers, health systems, professional associations, community-based organizations, employers, and institutions of higher education participating in project activities. 
  • Conduct academic detailing, educational outreach, and technical assistance activities to support the implementation of evidence-based practices related to opioid prescribing, substance use disorder prevention and treatment, harm reduction, and recovery support services. 
  • Assess organizational and provider needs through stakeholder interviews, gap analyses, workflow assessments, and environmental scans to identify opportunities for improvement and targeted support. 
  • Coordinate and facilitate provider education, stakeholder meetings, learning collaboratives, workgroups, webinars, and community engagement activities. 
  • Assist in the development, review, adaptation, and dissemination of educational materials, toolkits, implementation resources, and evidence-based curricula. 
  • Support implementation of interventions related to opioid stewardship, PMP utilization, overdose prevention, naloxone education, motivational interviewing, stigma reduction, and care coordination. 
  • Collaborate with clinician champions, state agencies, healthcare organizations, and community partners to advance project goals and improve adoption of best practices. 
  • Collect, track, analyze, and report project-related data, participation metrics, educational outcomes, and stakeholder feedback using approved reporting tools and systems. 
  • Assist with implementation and evaluation of quality improvement initiatives utilizing evidence-based methodologies, including workflow redesign, goal setting, and PDSA cycles. 
  • Ensure compliance with federal, state, organizational, privacy, security, and contractual requirements. 
  • Represent the organization at meetings, conferences, professional events, and stakeholder engagements as assigned. 
  • Maintain documentation of activities, technical assistance, educational sessions, and project outcomes to support contract reporting requirements. 
  • Participate in other public health, quality improvement, provider engagement, and educational initiatives as assigned.

  • Excellent interpersonal, verbal, written, presentation, and public speaking skills with the ability to effectively engage healthcare providers, community stakeholders, academic partners, and public health agencies. 
  • Strong facilitation and relationship-building skills, including the ability to establish credibility and maintain productive partnerships with diverse audiences. 
  • Knowledge of substance use disorders, opioid stewardship, harm reduction strategies, overdose prevention, stigma reduction, recovery support services, and evidence-based treatment approaches preferred. 
  • Understanding of quality improvement methodologies, change management principles, and implementation science concepts. 
  • Ability to analyze information, identify barriers, develop recommendations, and implement practical solutions. 
  • Experience delivering education, training, facilitation, academic detailing, or technical assistance in healthcare, public health, or community settings. 
  • Strong project management, organizational, and time management skills with the ability to manage multiple priorities and deadlines. 
  • Ability to work independently while contributing effectively within multidisciplinary teams. 
  • Experience collecting, tracking, analyzing, and reporting project and performance data. 
  • Proficiency with Microsoft Office applications, virtual meeting platforms, project management software, databases, and reporting tools. 
  • Knowledge of healthcare delivery systems, public health programs, healthcare quality improvement, and community-based service networks preferred. 
  • Demonstrated commitment to health equity, cultural competency, and serving diverse populations. 
  • Ability to travel for on-site meetings, educational events, stakeholder engagement activities, and project implementation support.

Minimum Experience Required:

Minimum of three to five years of experience in healthcare quality improvement, provider education, substance use disorder initiatives, practice facilitation, stakeholder engagement, healthcare consulting, project management, or related healthcare experience. 

Preferred experience includes: 

  • Provider outreach and engagement. 
  • Academic detailing or clinician education. 
  • Quality improvement and practice transformation initiatives. 
  • Substance use disorder, behavioral health, opioid stewardship, or harm reduction programs. 
  • Working with healthcare providers, health systems, FQHCs, community organizations, state agencies, or academic institutions. 
  • Planning, implementing, and evaluating grant-funded or contract-driven public health programs. 

Substitution(s):

Five years of relevant EHR, healthcare quality improvement, population health, and/or practice administrator experience may substitute for bachelor’s degree.
