Qualifications:
- Must successfully complete the required pre-employment Background and Drug Screenings
- Must possess a valid Driver's License which meets company standards for approved driver status
- Must possess basic computer skills
- Must possess a Bachelor’s Degree in the Human Services field
- Must have at least one year experience working in the field of intellectual/developmental disabilities
- Experience in writing program plans, case notes or other relevant documentation preferred
Essential Functions:
- Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires
- Creates positive, collaborative communication with all disciplines and team members
- Ensures that Program Plans and documentation are completed in accordance with all regulatory agencies
- Assists in monitoring and training of staff and relevant parties to ensure accurate, implementation and documentation of training, support and behavior programs as required by the individuals’ plans
- Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare
- Performs all other duties as assigned
