What you’ll do

  • Lead support planning and coordinate Support Workers to deliver person‑centred care

  • Manage caseloads, balancing safeguarding, admin, meetings and team guidance

  • Support customers with daily living skills, including budgeting, cooking and appointments

  • Work with multi‑agency partners to safeguard, empower and support move‑on into the community

You have

  • Experience supporting others, ideally within supported housing

  • Ability to assess referrals and coordinate support

  • Self‑motivated, resilient and calm under pressure

The practical bits

  • Rota-based role, Monday–Sunday, with alternate weekends

  • Core hours with flexible shifts between 8am–10pm

  • Based mainly in supported accommodation, with community visits across Hartlepool

  • Enhanced DBS with barring list required (paid for)

  • Full driving licence and business‑insured vehicle required (mileage paid)
