Job Title:  Team Case Assistant-Home Care

Department:  Home Care

Location: 330 Monroe Avenue Rochester, NY 14607

Hours Per Week:  24 hours per week

Schedule:  Friday-Sunday 7:30am-4:00pm

Position Summary:
Coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.


Key Responsibilities:
•   Ā Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met.
•   Ā Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee.
•   Ā Checks email in a timely manner providing follow up to assure sender’s needs are attended appropriately.Ā 
•   Ā Collects, organizes and reviews various programs for information and reporting and initiates follow up.
•   Ā Updates the EMR per agency standards.Ā 
•   Ā Performs functions related to transfers or discharges as required by regulation.Ā 
•   Ā Builds and maintains strong, communicative relationships with the team.
•   Ā Provides exemplary customer service.
•   Ā Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.Ā 
•   Ā Assists in orientation of new staff, role modeling, participates in quality assurance activities.
•   Ā Attends team meetings and other internal meetings as requested.Ā 
•   Ā Other duties as assigned

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
