By transforming the character, we give life back to the people who have lost it through substance abuse and mental health issues. Position Summary: Provides Substance Use Disorder (SUD) counseling and education for patients and their families across levels of care. Responsible for case management and collaboration with clinical staff, utilization management, and referral sources to support comprehensive patient care. Essential Job Functions Develop, implement, and regularly update individualized treatment plans Provide individual, group, and family counseling sessions Facilitate psychoeducational groups related to addiction, relapse prevention, and recovery skills Monitor client progress and modify treatment plans as needed Maintain accurate, timely, and compliant clinical documentation Provide case management, including coordination of care and referrals to community resources Develop discharge and aftercare plans to support sustained recovery Participate in crisis intervention and relapse prevention planning Collaborate with multidisciplinary teams, including medical, psychiatric, and clinical staff Communicate effectively with referral sources and external stakeholders Ensure compliance with state, federal, and organizational policies and ethical standards Other duties assigned Minimum Requirements: Bachelor’s degree in a behavioral health, human services, or related field preferred Active Licensed Chemical Dependency Counselor (LCDC) credential in the state of Texas Knowledge of Texas HHSC regulations and documentation standards Experience providing counseling services for substance use disorders PM21 Qualifications
