SPIRITUAL CARE COORDINATOR   Per Diem position 10-20 hrs per week.

 EXPOSURE CATEGORY I

BASIC PURPOSE & SCOPE: 

Under the supervision of the Spiritual Care Supervisor, is responsible for, but not limited to, spiritual counseling and direct spiritual care to patients and families as requested, the provision of consultation services to staff, and liaison activities with the religious community to facilitate the availability of spiritual care and understanding of special needs associated with hospice and home care.

RESPONSIBILITIES:

  1. Coordinates the involvement of spiritual care in preparing and updating patient/family care plans.
  2. Provides counseling to patients and family members to assist with their psychosocial adjustments to serious illness, death and grief.
  3. Serves as a member of the interdisciplinary team providing counseling to staff and volunteers related to their involvement with dying and grieving persons.
  4. Contacts clergy of a particular faith on the behalf of families as may be necessary.
  5. Provides direct spiritual care to patients and families without religious affiliation when requested.
  6.  Maintains liaisons with community organizations, particularly religious bodies, and serves as a resource for education.
  7.  May conduct funerals, baptisms, weddings as qualified and as may be requested by patient families. 
  8. Prepares required reports and documentation.
  9. Adheres to and reflects organizational values in daily work.
  10.  Serves on agency committees as may be assigned.
  11.  Maintains an obligation to report wrongdoing/violation of agency policies, applicable federal, state, and local laws, and rules and regulations pertaining to agency operations, to immediate supervisor or identified compliance officer.
  12. Completes all mandatory inservice education programs and completes any other additional inservice hours that are minimally required for the position.

REQUIREMENTS:

  1. Bachelor's degree in a related field from a regionally accredited college or university, or one  recognized by the N.Y.S. Education Department.
  2. One-year pastoral/spiritual work experience and training, preferably within a health related or similar setting.
  3. Completion of at least 1 unit of CPE (clinical pastoral education) preferred. Experience with terminally or seriously ill patients and families preferred.
  4. Demonstrated organizational skills.
  5. Effective oral and written communication skills.
  6. Computer literacy.
  7. Acceptable driver's license and use of automobile during work hours.
  8. Other requirements (physical) – see attached.
