Active Georgia PT or OT license in good standing
One or more years of clinical experience; home care experience preferred
Professional liability insurance carried by the contractor
Individual NPI number
Current CPR/BLS certification
Reliable transportation and a valid driver’s license
Background check and license verification before the first assignment
