The Housing Medical Case Manager I (MCM) will serve as part of UofL Ryan White Kentucky Care Coordinator Program (KCCP) case management team. The Housing MCM will provide housing-focused case management and advocacy services to Ryan White-eligible clients across the KIPDA region. This position supports clients in obtaining and maintaining stable housing while coordinating comprehensive, client-centered care that promotes treatment adherence, self-sufficiency, and improved health outcomes. Services are delivered in compliance with Ryan White Program requirements and University of Louisville policies.
Essential Duties and Responsibilities
- Maintain an active caseload based on state-mandated caseload standards by providing housing-focused, non-medical case management, treatment adherence counseling, and ongoing care coordination.
- Conduct comprehensive eligibility, acuity, housing, and needs assessments (including VI-SPDAT), develop individualized care and housing plans, and complete required eligibility recertifications every six months.
- Provide end-to-end housing navigation services, including identifying housing options, assisting with applications, budgeting, landlord engagement, moving assistance, emergency housing support, and referrals for long-term housing resources.
- Coordinate housing transitions and monitor client progress to ensure successful implementation of individualized housing and service plans.
- Collaborate with internal staff, behavioral health providers, community agencies, landlords, and housing partners to ensure coordinated care and successful linkage to housing, mental health, substance use, and other supportive services.
- Develop and maintain community partnerships to expand housing opportunities and connect clients with educational, vocational, employment, and independent living resources.
- Maintain accurate, timely documentation in EPIC, CareWare, and other required systems, including service encounters, eligibility documentation, housing activities, referrals, and required reports.
- Monitor housing referrals, track client engagement and outcomes, identify barriers to housing placement, and provide timely follow-up to support successful service linkage and retention.
- Participate in clinical quality improvement activities, performance monitoring, Ryan White compliance initiatives, and ongoing professional development related to HIV care and housing services.
- Perform other duties as assigned.
Preferred Qualifications
- Experience conducting VI-SPDAT assessments.
- Two (2) or more years of experience working within HIV care or Ryan White-funded programs.
- Bilingual proficiency in Spanish, Somali, Nepali, or another language commonly spoken by the client population.
Compensation will be commensurate to candidate experience.
