JOB SUMMARY/OBJECTIVE: Â
The Licensed Practical Nurse (LPN) provides comprehensive care to all assigned patients underÂ
the supervision of the Nurse Manager, in accordance with the Maryland State Nurse Practice ActÂ
and agency policies and procedures. The LPN is responsible for delivering quality care, makingÂ
clinical decisions, and adhering to regulations set forth by the Maryland Department of HealthÂ
and Mental Hygiene.Â
ESSENTIAL DUTIES AND RESPONSIBILITIES:Â
• Patient Follow-Up: Conduct follow-ups for patients requiring medical attention based onÂ
reviews of medical files and documentation, ensuring timely care.Â
• Medication Administration: Administer medications to clients as prescribed, ensuringÂ
safe and secure storage of all medications per state and federal guidelines.Â
• Regulatory Compliance: Maintain current knowledge of state standards for nursingÂ
practice in an intermediate care facility and ensure compliance at all times.Â

• Nursing Assessment: Perform comprehensive nursing assessments for each client uponÂ
admission and develop discharge care plans for ongoing medical conditions.Â
• Staff Orientation: Assist in the orientation of nursing staff as directed by the NurseÂ
Manager, fostering a culture of compliance and patient safety.Â
• Record Maintenance: Ensure all medical forms and clinical records are completedÂ
accurately and in a timely manner, in accordance with HIPAA regulations.Â
• TB Testing Records: Maintain accurate records of tuberculosis testing for all employees,Â
following state health regulations.Â
• Patient Documentation: Review and complete necessary patient paperwork as requiredÂ
to maintain compliance with healthcare standards.Â
• Incident Monitoring: Monitor and address client complaints and incidents, followingÂ
agency and regulatory guidelines for complaint tracking.Â
• Infection Control: Oversee and report on Infection Control/Surveillance activities toÂ
ensure a safe environment for clients and staff.Â
• Client Care Records: Maintain a comprehensive system of client care records, reportingÂ
forms, and service data in accordance with state and federal requirements.Â
• Service Coordination: Ensure all client care services are coordinated and integratedÂ
appropriately among multidisciplinary teams.Â
• Clinical Documentation: Keep necessary clinical documentation from physicians,Â
nurses, and other healthcare providers current and accessible.Â
• Collaboration: Work collaboratively with other healthcare professionals to provideÂ
holistic care.Â
• Additional Duties: Accept and perform tasks assigned by the Medical Director,Â
Executive Director, or their designee that are appropriate to this role.Â
• Meetings: Attend required meetings and trainings to remain informed and compliantÂ
with best practices.Â
• Other Duties: Perform all other duties as assigned.Â
REQUIRED EDUCATION, CERTIFICATIONS AND/OR EXPERIENCE: Â
• High school diploma or GED requiredÂ
• Valid Maryland LPN license required Â
• BLS certification is required.Â
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Â
• Ability to conduct ongoing chart reviews to prepare audits and ensure compliance withÂ
state and federal regulations.Â
• Demonstrated commitment to maintaining client confidentiality and respecting clients'Â
rights.Â
• Strong adherence to professional standards of practice, including punctuality, reportingÂ
protocols, and professional conduct.Â
• Consistent execution of monitoring tasks as outlined in Infection Control Policies.Â
• Ability to train and mentor staff in medication administration and clinical procedures.Â

• Prolonged periods of sitting at a desk and working on a computer.Â
• May require prolonged periods of standing, moving, and bending.Â
• Must be able to lift up to 75 pounds at times.Â
