Job Purpose:

The LPN functions under the supervision and guidance of the Clinic Manager, RN Case Manager, RN Team Lead, and/or Provider(s). Participates in the multi-disciplinary process of providing care and treatment to patients. They provide clinical support and technical assistance to the professional medical and other staff. The LPN is responsible for functioning within the scope of practice dictated by the authority of the Louisiana State Board of Practical Nurse Examiners. Individual must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with patients and their families.

Job Description:

  • Coordinate patient care by serving as a patient advocate, scheduling appointments, managing referrals, responding to requests, and facilitating communication between patients and providers.
  • Assist providers with patient visits by rooming patients, obtaining health histories, documenting information in the electronic medical record (EMR), and monitoring/reporting vital signs and clinical concerns.
  • Perform clinical support duties, including medication administration and documentation, point-of-care testing, specimen collection and processing, nursing care coordination, and triage support.
  • Educate patients and families on preventive health, medication adherence, appointment compliance, chronic disease management, and pediatric health and safety topics.
  • Manage administrative and clerical functions, including maintaining patient records, processing referrals and authorizations, handling prescription refills, coordinating medical records, confirming appointments, and supporting daily clinic operations.

Required Education:

  • Completion of state-approved practical nursing program

Skill, Licensure and Knowledge Requirements:

  • Current LA LPN License required
  • 3-5 years of primary care experience in a clinical setting (preference may be given to those with prior work experience in infectious disease and/or HIV depending on position)
  • Current Basic Life Support (BLS) Certification required
