Step Away from the Bedside — And Step Up to Next Level Care Coordination

Are you an experienced nurse looking for a meaningful career shift without leaving the clinical world behind? Join our Hospitalist team as a Patient Throughput Specialist (PTS) — an office-based role where your nursing expertise supports hospital operations, patient safety, and care continuity behind the scenes.

As a Patient Throughput Specialist, you’ll coordinate patient movement during inpatient stays, ensuring patients are placed with the right care team at the right time. You’ll use EPIC and other tools to remotely triage patients, manage consults, and support seamless hospital operations — all while collaborating closely with the Transfer Center, Emergency Department, and consulting services.

This role is perfect for nurses who love clinical reasoning, enjoy working with data and systems, and want to remain connected to patient care without the physical demands of bedside nursing.

What You’ll Do:

  • Receive, organize, and distribute incoming patient consults
  • Perform brief chart reviews to assess severity of illness and triage appropriately
  • Identify and document primary care physicians for care continuity
  • Review recent medical visits to support appropriate planning
  • Coordinate incoming patient transfers and referrals with the Transfer Center
  • Maintain strong communication with ER teams for triage and referrals
  • Run system queries and reconcile patient lists to ensure accurate tracking
  • Assign city call patients to academic teams appropriately
  • Generate daily reports on census, admits, consults, discharges, transfers, and deaths
  • Prepare night cross-cover materials to support safe handoffs per Joint Commission standards
  • Contribute to quality improvement initiatives through ongoing data collection
