Requirements:

Responsibilities:
• Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.
• Provide direct patient care services under the direction of a Physical Therapist.
• Assist the Physical Therapist in performing assessments and complex treatment procedures.
• Maintain documentation on all patients referred for services.
• Complete reports or assessments/evaluations and progress notes utilizing facility's approved documentation forms.
• Attend and participate in inter-disciplinary team meetings.
• Complete all tasks within established timelines provided by the facility.
• Comply with all state requirements and facility policies and procedures.
• Additional duties as assigned by facility.
