Job Purpose

As a Personal Advisor within the Leaving Care Service, you will manage a complex and diverse caseload of young people, supporting care leavers in their transition to adulthood and independence. You will undertake assessments, develop outcome-focused pathway plans, coordinate support services, and work collaboratively with professionals and partner agencies to promote positive outcomes for young people.

Key Duties/Accountabilities

  • Manage a complex and diverse caseload of care leavers and young people.
  • Undertake needs assessments and develop support plans based on identified needs.
  • Produce and review pathway plans in line with statutory guidance and procedures.
  • Support young people to achieve independence and positive transitions into adulthood.
  • Work with current and former asylum-seeking young people, those with unresolved immigration status, and young people with disabilities.
  • Build trusting and supportive relationships with young people and their families.
  • Coordinate services and support from partner agencies and professionals.
  • Promote participation and involvement of care leavers in decision-making.
  • Produce clear, accurate, and timely reports and records.
  • Support care planning and contribute to achieving positive outcomes.
  • Work independently while seeking guidance and support where appropriate.
  • Maintain professional boundaries and engage in continuous professional development.

Essential Experience Required

  • Experience working with children, young people, or vulnerable adults within a social care, support, housing, youth work, or community setting.
  • Experience managing complex caseloads.
  • Experience undertaking assessments and developing support plans.
  • Experience working in partnership with multiple agencies and professionals.
  • Experience supporting young people to achieve positive outcomes and independence.
  • Experience producing accurate reports, records, and case documentation.
  • Understanding of safeguarding principles and responsibilities.
  • Ability to build positive and trusting relationships with vulnerable young people.

Essential Qualifications Required

  • Relevant qualification in Social Care, Youth Work, Health & Social Care, Community Work, or equivalent relevant experience.
  • Enhanced DBS with Adults' and Children's Barred List.
  • Full UK Driving Licence.
  • Strong written and verbal communication skills.
  • Good IT skills and experience using case management systems.

Additional Information to Note

  • Working hours: 37 hours per week.
  • Working pattern: Monday to Thursday 08:45 - 17:00, Friday 08:45 - 16:00.
  • Location: Guildhall, Shropshire Council.
  • Enhanced DBS with Adults' and Children's Barred List required.
  • Full UK Driving Licence required.
  • Contract Length: 12 weeks with possible extension.
  • We work on bi-weekly payment schedule.
