Job DetailsJob Location: Raymond Clinic - Boise, ID 83704Education Level: RNJob Shift: DaytimeFull Circle Health is looking for a Clinical Case Manager to support our Raymond Clinic.PurposeThe purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. The Clinical Case Manager is responsible for identifying, screening, assessing, planning, implementing, coordinating, monitoring, and evaluating the healthcare services provided to patients at Full Circle Health.Schedule8:00am – 5:00pm, Monday - Friday daytime shifts.Benefit InformationWe offer a well-rounded benefits package to include everything you would expect Multiple health and dental plans No cost to employee life insurance, long term disability, employee assistance program, and financial advisors A variety of other optional benefits 6.5 paid holidays annually and PTO starting at 19 days per year  Retirement program with auto enrollment and matchQualificationsEducation:•    A.D.N. or B.S.N. from accredited College of Nursing, or a Masters in Social work from an accredited university. Experience:•    Minimum of two years of clinical practice; previous population health, case management or transitions of care experience preferred.•    Pediatrics experience preferred but not requiredLicensure / Certification:•    Valid license to practice as a Registered Nurse (RN) or LPN or Master's Prepared Social Worker in the state of Idaho, current BCLS certification, and Certified Case Manager certification preferred.Other Our organization is a tobacco-free workplace.
