Job Details: Job Location: Boise Pediatrics - Boise, ID, Position Type: Full Time, Salary Range: Undisclosed, Job Shift: Daytime, Job Category: Health Care, Full Circle Health is looking for a Pediatrics Case Coordinator to join our team!PurposeThe Pediatric Case Coordinator coordinates comprehensive, family-centered care for children and adolescents with complex health and social needs. Working collaboratively with patients, families, providers, schools, and community partners, this role develops and manages individualized care plans, facilitates access to healthcare and community resources, addresses barriers to care, and supports care transitions to improve health outcomes, continuity of care, and overall well-being.
