Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses (LPN) to perform home health visits in the Wellington/Loxahatchee, FL area. This role is critical in initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients.
- Provide home-based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility
- Examine patients, complete clinical assessments, document progress, and report changes to physicians
- Complete accurate and timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes
- Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives
- Deliver skilled nursing services and provide case management for assigned patients
- Educate patients on plan of care, medications, therapies, diet, and exercise
- Coach and support family members on patient care, medication management, and home safety
- Coordinate care with physicians, discharge planners, and interdisciplinary teams
- Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care
- Monitor patient progress, outcomes, and changing needs, adjusting care plans accordingly
- Ensure care goals are met and coordinate appropriate discharge from services
- Serve as a liaison with community health resources, including DME providers, oxygen services, and infusion therapy providers
- Maintain ongoing communication with patients, families, physicians, clinical managers, and office staff
- Participate in interdisciplinary team meetings and staff conferences
- Engage in continuous education and professional development opportunities
- Develop and follow work methods and procedures that support high-quality patient care
Requirements
Qualifications
- Active Florida Licensed Practical Nurse (LPN) in good standing
- Minimum 1 year of home health experience required
- Strong experience with OASIS documentation
- Experience with KanTime EMR system preferred
- Excellent clinical assessment and critical thinking skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license
