Job DetailsJob Location: Burlington, MA 01803Salary Range: $25.50 - $28.00 HourlyHOURS PER WEEK:     35        GENERAL SUMMARY:   The Care Manager is responsible for maintaining a caseload of older adult and individuals with disabilities who live in the community. Responsibilities include conducting home visits, assessing consumer needs, determining program eligibility, developing and coordinating service plans, and updating consumer records in a timely manner. Working as part of an interdisciplinary team, the Care Manager collaborates with agency nurses and community nursing facilities. ESSENTIAL FUNCTIONS: Conduct in-home functional assessments of consumers. Develop service plans with the consumer and their support network and monitor their ongoing effectiveness. Arrange services through contracted vendors and other community resources.  Assess consumers for risk, abuse and neglect and report such issues as needed. Maintain case documentation in accordance with AGE documentation standards. Assume responsibility for covering Care Manager duties several times per month assisting consumers whose Care Manager is unavailable. Participate in Staff Development including staff meetings and trainings. COMPETENCIES NEEDED: Accountability:  Demonstrates a high level of commitment to achieving results. Customer Service Orientation:   Learns internal and external customer needs and how best to meet them.  Responds to requests in a timely and thorough manner; does what is necessary to ensure consumer satisfaction; prioritizes consumer needs. Communication Skills: Listens, speaks, and writes clearly and concisely; ensures all involved are kept informed about issues or changes; keeps supervisor informed about progress and problems. Critical Thinking and Problem Solving: Recognizes the signs of a problem and utilizes available resources to resolve; analyzes procedures for possible improvements. Teamwork and Cooperation:  Demonstrates willingness to work with others toward a common goal. Time Management:  Organizes and schedules people and tasks to balance requisite number of home visits per month with maintenance of notes/calls/emails. WORK ENVIRONMENT: The employee will travel by car to meet with consumers where they live. Employees work in varied residential environments based on assigned caseload. The employee may be required to stand for long periods of time, navigate potentially unsafe environments, and navigate stairs to enter consumer homes. This role regularly requires the use of a computer, telephone and other standard office equipment.  PHI and PII: This role has access to Protected Health Information (PHI) and therefore the employee must adhere to Minuteman’s confidentiality policy and HIPAA regulations. This role has access to Personal Identifiable Information (PII) and therefore the employee must adhere to Minuteman’s confidentiality policy and MA Executive Order 504. Pay Range Disclosure: In accordance with the Massachusetts Wage Transparency Act, the pay range for this position reflects what we reasonably and in good faith expect to pay at the time of posting. Final compensation will depend on the candidate’s experience, skills, and alignment with the role’s responsibilities. QualificationsQUALIFIED APPLICANTS WILL HAVE: Bachelor’s degree in human services, social work, or related field preferred. Equivalent work experience may be considered in place of degree.  Experience utilizing MS Office including Word and Outlook email and calendars. Experience working with the older adult and disabled population including those with behavioral health/substance misuse issues is preferred. A reliable car, valid driver's license and car insurance are required. Prior care management experience a plus
