This role is a Daylight (8am-4pm), Monday - Friday with the possibility of an evening or weekend day occasionally.   Essential Functions: Conduct comprehensive assessments and develop a treatment plan for consumers to determine needs and resolve barriers. Outreach to all providers, agencies, and entities to educate and inform of services provided by the SSVF program and to solicit referrals to program. Screening of all referred veteran individuals/families to determine eligibility for the program. Assessment of needs for all Veterans and families eligible for SSVF services and supports. Case management for all eligible veterans and families, including housing-focused case management to ensure stable housing plan. Connecting all eligible Veterans and families to appropriate VA resources/benefits. Connecting all eligible Veterans to mainstream resources and benefits based on the assessment of needs. Ensure timely and accurate documentation of all case management supports and services provided.Qualifications: High School Diploma or GED 1 year experience in social services field, Experience working with the homeless population; Computer literacy Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required.
