Job Details: Job Location: Carbondale, PA 18407-2304, Salary Range: $19.50 - $22.90 Hourly, $2,000 Sign on BonusBlended Case Manager (BCM), will promote resiliency and recovery for children, adolescents, adults and their families by accessing, coordination, mentoring, and advocating for community resources and services.Qualifications: Bachelor’s Degree in Human Services or related field.  Must work 40 hours per week, have flexible schedule, and on-call is required.
