Responsibilities
- Provide comprehensive primary care to patients across the lifespan
- Perform wellness visits, physicals, acute visits, and follow-up care
- Evaluate, diagnose, and manage acute and chronic conditions
- Provide preventive health counseling and patient education
- Coordinate care with physicians, specialists, and other members of the healthcare team
- Maintain accurate and timely electronic medical records

Community Care Physicians (CCP) is a physician-led, independent multispecialty medical group serving New York’s Capital Region. CCP provides administrative, clinical, and technological support to help providers focus on patient care while maintaining the collaborative feel of a community-based practice.
- License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- Physician Assistant:
- Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
- Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
- Current certification by National Commission on Certification of Physician Assistants.
- At least one year of experience as a physician assistant in ambulatory health care.
- Current CPR certification.
- Nurse Practitioner:
- Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
- Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
- Successful completion of pharmacology coursework acceptable to NYSED.
- Three years of professional nursing experience in an ambulatory setting preferred.
- At least one year of experience as a nurse practitioner in ambulatory health care.
- Current CPR certification.
