As the Case Manager you will be responsible for the planning and treatment of individual children, a family, or assigned groups by performing a variety of regular tasks. Specific responsibilities include: Developing a care plan for each child containing goals and objectives with projected dates of accomplishment  Involving the clients, referral agency and family in planning whenever possible Developing a specific plan relating to the involvement of the child’s parents. Participating in the documentation of case assessment. Documenting the implementation of the care plan. Providing for scheduled in-person conferences with each resident. Providing a supportive atmosphere for the youth with leadership and guidance to the client. Coordinating internal and external activities of the youth. Serving as a liaison with the referring agency. Providing a mechanism for dealing with day-to-day program operations. Oversees and maintains general health and well-being of the child Overseeing the living activities of the child. Monitoring and recording behaviors on a daily basis.   Why Work here?
