Job Summary and Responsibilities

As our Transfer Coordinator, you will meticulously coordinate all processes, support, communication, and documentation involved in transferring patients between facilities and facilitating physician consultations. This includes connecting referring physicians with accepting/consulting specialists and coordinating bed availability with receiving facilities. You'll also be instrumental in arranging Flight for Life or other patient transports when necessary, ensuring every detail is managed with precision and efficiency.

 

Every day you will professionally and accurately represent all facilities, acting as a crucial liaison between referring and receiving physicians and hospital staff. You'll tirelessly develop and maintain strong referral relationships with outlying facilities through exceptional customer service. You'll manage a high volume of incoming calls using specialized dispatcher software, patching calls to physicians and facilities, and ensuring the "on-call" database is always up-to-date and accurate.

 

To be successful in this role, you will demonstrate exceptional customer service, strong organizational skills, and the ability to multi-task efficiently and effectively. You must be adaptable to rapidly changing technologies, policies, regulations, and staffing through proactive priority setting and flexibility. Your calm demeanor under high-stress situations, excellent computer and communication skills, and unwavering professionalism will be critical in ensuring smooth operations and optimal patient outcomes.

  • Coordinates requests for transfer of patients into the Health system, or requests for medical consultation, including connecting the referring MD with the accepting/consulting MD. Also coordinates with receiving facility bed availability for the patient and offering Flight for Life transport coordination if applicable.
  • Accurately document and enter all required information into the EPIC database as it occurs, for creation of real time data collection. Additionally, run reports every shift to identify and correct any missing data points, etc.
  • Facilitate all communications between referring and receiving physicians and other hospital staff, while relaying information accurately.
  • Professionally and accurately represent all facilities. Ability to develop and maintain referral relationships with outlying facilities through consistent exceptional customer service.
  • Ability to multitask while utilizing Windows-based console Dispatcher software and system for multiple incoming calls, patching with receiving physicians and facilities, etc.
  • Coordinate Flight for Life air and ground transportation as needed.

Job Requirements

Required

  • High School Diploma/G.E.D.
  • One (1) to two (2) years of direct clinical experience in Emergency Department/Critical Care Unit (such as Critical Care Tech EMTB etc.), OR one (1) to two (2) years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative), OR one (1) to two (2) years Transfer Center experience.
  • Working knowledge of medical terminology.
  • Certified Healthcare Access Associate (CHAA) successfully completed within 18 months of employment. Certification must be maintained while employed.
