House Calls Nurse Practitioner - Full Time - Company Car

What You’ll Do:

• Provide in-home primary care for chronic disease management, dementia care and preventive healthcare
• Perform cognitive screenings, annual wellness visits, follow-ups, and hospital-to-home transition supervision

What You Bring:

• Willingness to travel to patient’s homes
• Active Virginia Nurse Practitioner
• DEA registration or eligibility
• Medicare PECOS enrolled
• Experience in primary care and geriatrics
