                                              **CNA FACILITY COORDINATOR – FULL-TIME**

                                                                 LOCATION:  CHAPEL HILL

Allcare Home Health Agency is committed to delivering compassionate, dependable, and high-quality home care services. We are seeking an organized, dependable, and experienced Certified Nursing Assistant (CNA) to serve as our Facility Coordinator.

The CNA Facility Coordinator will oversee and coordinate the delivery of assigned home care services to clients residing in independent and assisted living facilities. This individual will serve as the primary liaison between Allcare Home Health Agency, facility staff, caregivers, clients, and families to ensure effective communication, appropriate staffing, quality service delivery, and client satisfaction.

Responsibilities:

Facility Coordination and Relationship Management

• Serve as primary agency contact at the facility
• Establish and maintain positive working relationships with the facility administrators, directors of nursing, and other facility personnel.
• Monitor service delivery and maintain communication.
• Address facility concerns and coordinates timely resolutions.
• Promote Allcare Home Health Agency services and maintain strong professional relationships with facility partners.

CNA and Caregiver Coordination

• Coordinate caregiver assignments and staffing schedules for clients in assigned facilities.
• Monitor caregiver attendance, punctuality, and shift coverage.
• Assist with caregiver orientation and facility-specific expectations.
• Report staffing concerns and performance issues to management.
• Assist clients with personal care, including bathing, dressing, grooming, and toileting
• Medication reminders
• Follow individualized care plans and agency policies
• Vital Sign checks

Client Care and Service Monitor

• Conduct routine client check-ins and service satisfaction visits.
• Monitor whether assigned services are being provided according to the approved service plan.
• Communicate client concerns, complaints, and changes in condition to the appropriate Agency’s Director of Nursing.
• Ensure concerns requiring clinical assessment are promptly referred to the Director of Nursing or designated clinical personnel.
• Maintain regular communication with clients, families, and facility representatives.
• Promote client safety, dignity, and satisfaction.

Qualifications & Requirements:

• Current NC CNA Certification
• Minimum of 5yrs working as a CNA
• High school diploma or equivalent preferred
• Experience working in assisted living or independent living facilities are preferred.
• Previous supervisory, scheduling, or coordination experience is a plus.
• Excellent organizational and time-management skills.
• Excellent communication and interpersonal skills
• Compassionate, dependable and professional demeanor and ability to establish positive working relationships.
• Ability to use computers, email, scheduling systems, and electronic documentation platforms.
• Must successfully complete applicable background checks and agency screening.
• Current TB Test
• BLS Certification (American Heart Association)
• Reliable transportation and valid driver’s license
• Current Auto Insurance
• Compassionate, dependable and professional demeanor

Physical Demands:

• Combination of sitting, standing, bending, reaching, stretching, stooping, walking, climbing stairs and moving intermittently during working hours.
