DIRECT CARE PROFESSIONAL

SUMMARY

Provides supervision and support to adults with disabilities by performing the following duties and in accordance with their Person Centered Plan (PCP).

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

▪Advocates for individuals supported by Life Center, Inc

▪Adheres to emergency program procedures for fire, tornadoes, accidents, or other serious incidents

▪Administers medications to persons supported as indicated by state guidelines, physician direction, and the Person Centered Plan

▪Prepares meals according to Person Centered Plan and physician recommendations

▪Implements all aspects of the Person Centered Plan

▪Documents all required information before end of work shift, i.e. progress notes, time sheets, and incident reports and ensures that all documentation is complete and signed before submitting

▪Drives vehicle in a safe manner adhering to all traffic regulations

▪Reviews mileage documentation for accuracy and submit be designated due date

▪Provides transportation to the individual supported as indicated in the Person Centered Plan and/or as requested by the individual

▪Provides transportation to person supported to and from meetings, appointments, and all other community activities with the use of employees personal vehicle when program vehicle is not available

▪Participates in community activities chosen by the individuals supported

▪Attends all staff meetings, training sessions

▪Reads the Program Log Book every shift. Initials each entry as it is reviewed

▪Complies with Life Center policies and procedures

▪Assists persons supported in making purchases in the community

▪Models appropriate social behavior for persons supported

▪Demonstrates ability to make sound decisions

▪Follows assigned work schedules

▪Completes all scheduled shift responsibilities including cleaning tasks

▪Assists supervisor in routine site maintenance

▪Identifies self when accepting incoming telephone calls to facilitate effective team communication

▪Provides supervision to the persons supported according to the supervisory guidelines established by the Person Centered Plan

▪Works cooperatively and professionally with family members, guardians, and stakeholders

▪Demonstrate effective verbal and written communication

▪Works collaboratively with the Supports Coordinator and other clinical support staff

▪Communicates professionally with agency support staff

▪Observe safety and security procedures, utilize equipment and materials properly and report potentially unsafe conditions

▪Adhere to State of Michigan Licensing Regulations and the Michigan Mental Health Code

▪Completes all tasks delegated by immediate supervisor or designee

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

High school diploma or general education degree (GED) preferred; or one to three months related experience and/or training; or equivalent combination of education and experience.

LANGUAGE SKILLS

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

CERTIFICATES, LICENSES, REGISTRATIONS

Residential Direct Care training (applicable to program assignment) including: Reporting Requirements,

Medication Training/ Certification (if passing medications), Personal Care, Supervision and Protection,

CPR and First Aid Training /Certification

Infection Control – Prevention of Communicable Diseases Training/Certification

Fire Safety and Prevention Training

Recipient Rights Training at time of hire and annually

Valid Michigan Driver's License

Valid Vehicle Insurance

Any other training certificates and requirements by Life Center
