If you are a CRS/CFRS, or meet the requirements to pursue certification with the Pennsylvania Certification Board, this could be a great opportunity for you to  to help others in a unique professional role.   ESSENTIAL DUTIES AND RESPONSIBILITIES    Performs work associated with recovery support services, case management, treatment and rehabilitation of persons with drug and/or alcohol use.  Services and support will be delivered to patients in the primary care setting in conjunction with Medication Assisted Treatment (MAT).     In accordance with Patient Centered Medical Home philosophy, collaborates with team members including:  patient; primary care provider; behavioral health provider and others to deliver comprehensive care to patients of Family First Health through: Patient Engagement and Empowerment Participates in care coordination by utilizing case management tools, such as strength-based case management, motivational interviewing to facilitate patient retention in care and move patients toward independence and increased autonomy. Assists participants in the management of their recovery by ensuring that the resources to address the participant’s needs are in place. Provides drug and alcohol education, risk reduction support, and medication adherence guidance.  Promotes participant retention, self-sufficiency in treatment and utilization of available support through educating participants about service availability. Engages in brief relationship building and active listening with patients receiving treatment Provides supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery. Supports patients in developing a self-directed recovery plan to meet their recovery goals Will serve on-call duties based on on-call policy and departmental procedures. Connection with other providers  Identifies, develops and maintains positive linkages with agencies and social service organizations/services in the community and within FFH.  Communicates with FFH interdisciplinary team, peers in other departments, managers, patients and other community agencies to ensure ongoing care for patients, maintain connectedness, and foster positive internal/external customer service.  Facilitates warm handoffs between all available behavioral health and physical/dental health treatments.  Creates and manages long-term individualized service plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients.  Documentation   Maintains timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to program funding.   Understands and adheres to all state and federal drug & alcohol confidentiality laws and related statutes.  Attends and actively participates in both internal and external meetings as required.  Attends trainings and conferences as determined by supervisor.  Performs other duties as assigned and related to the position.  Qualifications Must have a minimum of two (2) years uninterrupted recovery Must have or be eligible to pursue Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) credential Must be actively engaged in activities that support a recovery oriented lifestyle High School Diploma or GED required. Previous education/experience with drug and alcohol programs preferred Excellent customer service skills with the ability to effectively establish appropriate boundaries  Strong written and verbal communication skills with the ability to support individuals of all backgrounds Ability to display critical thinking and exercise individual judgement and discretion in matters of significance  Ability to establish and maintain effective working relationships with patients, employees, and the public   Ability to be organized and work efficiently with strong attention to detail within a demanding work environment while appropriately prioritizing tasks Thorough knowledge of the Guiding Principles of Recovery.   Knowledge of MAT and ability to perform duties in the context of MAT.  Knowledge of computer software applications (Microsoft Office applications, health records, others)  Availability to work a 40 hour week between the hours of 7:00 am and 7:00 pm. Ability to set positive examples for team members through exceptional work performance including adherence to company policies and processes. Ability to work independently and within a multidisciplinary team Ability to work in, and respond appropriately to a demanding environment including crisis situations Ability to appropriately handle sensitive and confidential information Ability to be organized, proactive in follow-up and follow-through, and to manage multiple competing tasks Ability to adhere to all Family First Health’s policies, processes, and procedures Ability to articulate and advocate Family First Health’s mission and values. Ability to maintain required clearances and licensure. PHYSICAL DEMANDS  Mental/Physical Requirements: Must possess the physical and mental abilities to perform the tasks normally associated with a Recovery Specialist position such as:    Frequent and prolonged work at a computer and/or on the phone for extended periods of time with repetitive typing with arm and hand motion   Periodic travel between work sites with exposure to weather conditions   Requires correct vision and hearing to normal range  Occasional bending, stooping, crouching, or kneeling   Frequent standing, walking, turning, and reaching   Frequent contact with patients, clients, vendors, and visitors   Occasional carrying of office supplies and light equipment   Frequent competing demands, frequent changes, delays, or unexpected events   When teleworking or working remotely, requires working alone in a qualified work setting
