The Medical Social Worker Liaison collaborates with Ohio’s Hospice Admissions and Referrals Department and Care Team Coordination Department to ensure patients and families receive education, support, and guidance throughout the transition to hospice care.

Key Responsibilities

  • Work with patients and families experiencing complex psychosocial issues.
  • Engage patients and families in thorough discussions regarding goals of care, providing education on how hospice can support those goals and, when appropriate, identifying additional community resources.
  • Provide emotional support to patients and families as they navigate the challenges and crisis that may accompany end-of-life decisions.
  • Educate patients and families regarding available care setting alternatives, including home, long-term care, skilled nursing facilities, private-duty caregiving, and hospice houses.
  • Ensure patients and families understand the Medicare Hospice Benefit and facilitate informed consent.
  • Address insurance coverage and financial concerns, including completing financial assessments as appropriate.
  • Assist patients and families with advance care planning and advance directive needs.
  • Communicate and collaborate with hospital staff to establish coordinated plans of care that reflect the hospice philosophy for both new patients and existing Ohio’s Hospice patients hospitalized for any reason. Help facilitate timely transitions to appropriate care settings.
  • Assist with obtaining required hospice consents and related documentation.
  • Provide initial and ongoing comprehensive psychosocial assessments for patients in facility-based contracted beds.
  • Provide social work case management for Ohio’s Hospice patients admitted to the hospital for symptom management.
  • Serve as a resource and liaison for hospital partners throughout the Central Region, building strong relationships and supporting consistent coordination of hospice services across the region.
