Job DetailsJob Location: Columbus, OH 43215The RREACT program is seeking dedicated and compassionate Outreach Coordinators to work in partnership with local emergency departments to conduct follow-up visits with patients who have experienced an opioid overdose. The Outreach Coordinators will engage directly with these patients in the hospital to assess their needs and provide immediate referrals to detoxification services, addiction treatment programs, primary medical care, and other necessary resources. This role plays a crucial part in supporting patients' recovery and connecting them to vital services that aid in their long-term wellness. Responsibilities: Partner with local emergency departments to conduct follow-up visits with patients who have experienced an opioid overdose. Engage with patients to assess their needs and desires for treatment and recovery. Coordinate referrals to immediate detox services, addiction treatment programs, primary care providers, and other supportive services. Provide education and emotional support to patients regarding available treatment options. Collaborate with a multidisciplinary team to ensure a holistic approach to care and recovery. Maintain up-to-date knowledge of local resources and treatment options for opioid addiction. Document all interactions and follow-up activities accurately and promptly. Monitor patient progress and continue to offer support and referrals as needed throughout the recovery process. QualificationsQualifications: AA/BA degree required. CDCA or higher required.  Experience working in healthcare, addiction treatment, or outreach services preferred. Excellent communication and interpersonal skills, with the ability to engage with patients in a supportive and non-judgmental manner. Ability to work independently and as part of a collaborative, interdisciplinary team. Experience in crisis intervention and patient engagement.
