About Nexus Neurorecovery Center – Conroe

Our Conroe Neurorecovery inpatient rehab provides interdisciplinary, patient-centered care, focused on improving functional independence and quality of life for patients with neurological injuries and complex diagnoses.

The program at Conroe focuses on neurological rehabilitation, functional skill development, and stability in a structured, assisted-living rehab environment. Our interdisciplinary team works closely with residents and their families to promote independence, safety, and long-term progress through individualized treatment plans, therapeutic programming, and evidence-based care.

We serve individuals with traumatic brain injuries, strokes, spinal cord injuries, and complex neurological conditions who require ongoing rehabilitation and support to improve quality of life and daily functioning.

Position Summary

The Licensed Vocational Nurse (LVN) provides direct nursing care to residents with neurological and medically complex conditions under the supervision of a Registered Nurse. The LVN supports implementation of care plans, administers medications, monitors patient status, and collaborates with the interdisciplinary team to promote safety, stability, and functional progress.

This role requires consistency, strong attention to detail, and the ability to work with residents who may have cognitive, neurological, behavioral, or complex medical conditions in a structured rehabilitation environment.

Key Responsibilities

  • Provide direct nursing care under the supervision of a Registered Nurse
  • Administer medications and treatments per physician orders and facility policy
  • Monitor patient condition and report changes to the RN or provider
  • Assist with the implementation of individualized care plans
  • Support patients with neurological, behavioral, and medically complex needs
  • Maintain accurate and timely documentation in the electronic medical record
  • Follow Nexus policies, HIPAA standards, and infection control procedures
  • Use approved de-escalation techniques when needed to maintain safety
  • Collaborate with therapy, nursing, psychology, and medical staff
  • Support a structured, therapeutic environment focused on rehabilitation and stability

Qualifications

  • Active LVN license in the State of Texas required
  • BLS Certification required
  • ACLS preferred
  • De-escalation / behavioral management training required within 30 days of hire
  • Graduate of an accredited vocational nursing program required
  • Experience in rehabilitation, hospital, long-term care, or behavioral health preferred
  • Experience working with neurologic or medically complex patients strongly preferred
  • Ability to work effectively within a multidisciplinary clinical team
  • Dependable, calm, and patient-centered approach to care
