PATIENT INFORMATION
• Patient Demographics: Adults

• Case Mix: Adult primary care, population health, health equity, and value-based care

DUTIES (Teaching & Educational Leadership)
• Faculty for the Internal Medicine Health Equity Track
• Supervise Internal Medicine residents
• Clinical teaching in Primary care, Population health, Value-based care, Health equity-focused practice
• Teach residents to integrate in social determinants of health, community-engaged care approaches, equity-centered clinical decision making
• Participate in journal clubs, lecture series, case conferences, resident workshop
• Mentor residents in quality improvement initiatives, health equity research, scholarly activities/projects
• Collaborate with residency and program leadership to develop and improve curriculum
• Provide formative and summative feedback to residents to support professional growth

• Multidisciplinary team including care managers, pharmacists, social workers, behavioral health specialists, and community partners
