The Staff Physical Therapist is responsible for providing therapy services, upon referral from an appropriate licensed practitioner, to patients including assessment, treatment planning, and implementation related documentation and communication.  Evaluates and treats patients in order to produce effective outcomes.  Communicates and assists in resolving problems.  Demonstrates effective resource utilization that is critical to operational stability within the organization.  Participates in performance improvement and program growth.   

Responsibilities

•  Reports for scheduled shift on time and prepared to assume responsibilities.
• Attends and participates in meetings and is responsible for all information communicated at meetings.
• Adheres to Corporate, Regional, hospital and departmental policy and procedure 100% of the time.
•  Assists all patients, family members, physicians, visitors and employees in a professional and courteous manner.
• Adheres to the following environmental care standards.
• Performs and fulfills all HICS (hospital incident command system) responsibilities, assignments, tasks, and roles as directed and needed by the facility in times of emergency and/or disaster. Responsible for knowing the Hospital’s emergency codes, including the purpose and appropriate action for each, and adheres to the Hospital polices and procedures for each emergency code. 
• Demonstrates the ability to adapt to varied age-specific patient populations .
• Complies with all policies & procedures for this position that pertain to HIPAA.

Qualifications

EDUCATION:

• Minimum: Bachelor of Science Degree  
• Preferred:  Master’s Degree in Physical Therapy

EXPERIENCE:

• Experience desired, but not required

REQUIRED CERTIFICATION/LICENSURE/REGISTRATION:

• Florida license as a Physical Therapist from the Agency of Health Care Administration’s Division of Medical Quality Assurance.
• Current AHA BLS CPR card
