Now Hiring: Home Care & Hospice Registered Nurse (RN)

• Provide skilled nursing assessments and direct patient care in the home setting.
  
  
• Develop, implement, and evaluate individualized plans of care.
  
  
• Perform comprehensive patient assessments and monitor changes in condition.
  
  
• Provide medication management, wound care, disease management, and patient education.
  
  
• Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team.
  
  
• Provide compassionate hospice care, including symptom management and end-of-life support.
  
  
• Educate patients and caregivers regarding disease processes, medications, safety, and care needs.
  
  
• Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
  
  
• Communicate effectively with patients, families, physicians, and the clinical management team.
  
  

• Current, unrestricted Michigan Registered Nurse (RN) license.
  
  
• Previous home health, hospice, or acute care experience preferred.
  
  
• Strong clinical assessment and critical-thinking skills.
  
  
• Excellent communication and organizational skills.
  
  
• Ability to work independently and manage a flexible daily schedule.
  
  
• Valid driver's license, reliable transportation, and automobile insurance.
