SUMMARYÂ
This position collaborates with referral sources to coordinate the initiation of admission into the hospice program. Coordinates patient referral and identifies hospice appropriateness with documentation of terminal eligibility criteria. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships with referral sources to increase access to hospice services. Complies with all agency policies and legal rules and regulations.Â
KEY RESPONSIBILITIESÂ
Collaborates with a variety of referral sources to coordinate the transition from care from a facility or office into the hospice programÂ
Collaborates with attending physicians /medical directors to obtain the certification of terminal illnessÂ
Collaborates with hospice leadership team as well as field staff to ensure smooth transition into the hospice programÂ
Obtains appropriate physician orders both written and verbal for initiation of care into the hospice programÂ
Obtains order verification and approval as required by regulation and/or policyÂ
Updates physicians and referral sources of changing needs of patients prior to dischargeÂ
Notifies scheduling of patient needs; allowing for admission into the hospice programÂ
Participates in problem resolution as neededÂ
Assists agency staff, referral source, physician, and the patient in establishing a plan of care for the patient prior to admission to the hospice programÂ
Orders medical supplies and durable medical equipment as needed to facilitate the transition to homeÂ
Maintains contact with community referral sources to provide education and build relationshipsÂ
Develops and maintains relationships with acute care facilities, physician offices, skilled nursing facilities, long term care, rehab facilities to enhance referrals through phone, e-mail or in-person contact as appropriateÂ
Follows and coordinates care on active community patients who are admitted to a facility and return to homeÂ
Documents accurate and complete patient information in a timely manner per policies and proceduresÂ
Demonstrates financial prudence by minimizing waste of supplies, materials and timeÂ
Adheres to productivity standards as set forth by the CompanyÂ
Demonstrates behaviors consistent with the service excellence standards set forth by the CompanyÂ

QUALIFICATIONSÂ
Two years of hospice experience with 1 year as an RN Case Manager preferredÂ
Knowledge of Hospice Conditions of ParticipationÂ
Preferred competency with technology including but not limited to Microsoft Office, HCHB, Epic, and other Healthcare portalsÂ
Knowledge of Medicare/Medicaid RegulationsÂ
Demonstrates honesty; keeps commitments; behaves in a consistent mannerÂ
Maintains confidentiality of all patient informationÂ
Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisorsÂ
Experience with coordinating or managing discharge planning preferredÂ
Skilled in communication and time-management techniquesÂ
Detail oriented with ability to oversee multiple responsibilities and ability to prioritize responsibilitiesÂ
EDUCATION AND CREDENTIALSÂ
Registered Nurse with current license in the state of OhioÂ
Valid Driver’s license and proof of insurance requiredÂ
