SUMMARYÂ
The Speech Language Pathologist (SLP) provides comprehensive skilled care as ordered by the attending physician within specified geographical territory. The SLP educates patients and their family members and ensures the safety of the patient. The SLP is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The Company adopts the speech-language pathologist scope of practice as outlined in Chapter 4753 ORC for its licensed SLP personnel. The SLP is responsible for all practices and duties within the scope of practice as outlined by the state.Â
KEY RESPONSIBILITIESÂ
Provides skilled speech therapy as ordered by the attending physicianÂ
Evaluates the patient’s level of functioning by applying diagnostic and prognostic proceduresÂ
Develops an individualized ST plan of care and makes adjustments as needs changeÂ
Observes, records, and reports patient reactions, changes, and progress to attending physician and any appropriate agency staffÂ
Informs health care staff through certain phases of speech therapy which they may use to work with the patientÂ
Uses activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related functionÂ
Guides the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioningÂ
Promotes continuity of care with appropriate admissions, transfers and dischargesÂ
Counsels and instructs patient and family on the disease/injury/disability and how to manage in all phases of speech therapyÂ
Incorporates patient and family in development of plan of careÂ
Instructs patient and family members on proper use of procedures and any equipmentÂ
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsÂ
Listens to patient and family membersÂ
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesÂ
Documents all aspects of treatment, assessments, and patient educationÂ
Completes chart audits at agency location as requestedÂ
Communicates with scheduler any changes as neededÂ
Participates in all on-call requirements and case conferencesÂ
Attends in-service trainings and mandatory agency meetingsÂ
Participates in education programs and in-services as mandated by state lawÂ
Participates in orientation/training of new employeesÂ
Maintains a professional appearance as a representative of the companyÂ
Understands and practices agency policies and proceduresÂ
Other duties as assignedÂ
QUALIFICATIONSÂ
Minimum of on year of experience as a SLP is preferredÂ
Home care experience is desiredÂ
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workersÂ
Knowledgeable on speech therapy best practices. Ability to make appropriate speech therapy judgmentsÂ
Ability to identify a situation and handle it with the best possible solutionÂ
Detail-oriented and observantÂ
Disciplined style of work ethic with the ability to prioritize and be timelyÂ
Valid driver’s license and auto insurance with your name as a listed driverÂ
EDUCATION AND CREDENTIALSÂ
A current Speech-Language Pathologist (SLP) license in the state of employment.Â
