Clinical Integration Specialist – Palliative Care


Visiting Nurse Health System (Visiting Nurse) is seeking a Clinical Integration Specialist for our Palliative Care team who will serve as a strategic partner with the CEO, Executive Director, and Business Development leadership to promote and streamline internal referrals. The Clinical Integration Specialist (CIS) impacts the overall enterprise-wide clinical integration strategy, focusing on care coordination and collaboration, enhancing effective communication, and developing and implementing standardized protocols and guidelines. The CIS also fosters data sharing and analytics.

In this position...

  • Impacts the reduction in Home Health hospitalization rates through proactive care coordination and referrals
  • Utilize analytic data (Acclivity and Bamboo) and provide education to internal and external customers and stakeholders on the value of Hospice and Palliative Care.
  • Serve on the Clinical Integration Committee and participate in the committee’s effort to advance the strategic goals.
  • Increase internal Home Health referrals to Hospice and Palliative Care by 10% month-over-month.
  • Increase cross-departmental utilization of Palliative Care by 10% quarterly, with a baseline of 2.43% overall across all clinical units.
  • Meet the referral to admission conversion of 85%

 

Do you have....

  • Graduate of an accredited Practical Nurse Degree, Associate Degree, BSN in Nursing or MSW
  • Current RN or LPN License, or a Licensed MSW
  • 2-5 years of Hospice and Palliative Care experience. Prefer 1 year of clinical experience
  • Must have reliable transportation, a GA valid driver's license, and current vehicle insurance


Compensation: $65,600 - $98,400
