This role supports adults with chronic substance use histories who benefit from structured, supportive care. The position focuses on coordinating transitional residential services, providing case management and skills training, and supporting clients throughout their recovery journey. WHY WORK FOR SOUTHWESTERN? Evaluate and screen applicants appropriateness for admission to the program. Coordinate admissions to the service, as well as discharges from the program Monitor living environment, involvement in community living and self-help groups, employment, and payment of fees. Maintain adequate inventory of food and household supplies. Monitor utilization of services and adhere to quality care standards. Monitor residents compliance with transitional residential services guidelines. Coordinate reentry into the community. Provide skills training and care coordination  to Transitional Residential patients. Develop appropriate treatment plans and goals and monitor patient’s progress toward treatment goals. Coordinate patient care with other members of the treatment team. Maintain contact with referral sources and other providers as necessary. Maintain appropriate clinical documentation. Provide care coordination to clients in other addiction programming (residential and outpatient) as needed. Collaborate with nursing staff and collaborating physicians to ensure high quality of medical care for clients. Develop and sustain relationships with other providers in the community to provide for on-going client needs.Qualifications: MINIMUM QUALIFICATIONS: Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required. Experience working with adults with a substance use disorder is preferable.  Possess the ability to assess emergency situations and react in an appropriate and therapeutic manner. Must have valid driver's license and qualify for Company's vehicle liability insurance.
