Sage Group HealthCare Staffing is seeking Home Health PT for a 13-week travel/contract positions in Everett, WA. We are accepting both local and travelers candidates.

Pay Rate: $3000 total weekly gross - $75/hour($31/hour W - 2 taxable , $44/hour non taxable stipend)

Shifts available: Days
Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
- Physical Therapist: 1-2 years - Required
- Recent/Current Home Health OT experience: 1 year within the last 3 years - Required
- Home Care Home Base (HCHB) charting - Required
- Must have valid Driver’s License and Personal Auto Insurance
Education, License and Certification Requirements:
- PT License for State applying or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide Physical Therapy services for Home Health Patients
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on a daily basis.
- Complete charting and patient documentation in timely manner as per client policies and procedures
- Territories:50+ miles radius from the office in zipcode 98203 - Primarily Lake Stevens/Snohomish area
