We are searching for a talented Advanced Practice Practitioner to join our Health at Home – Supportive Care Hospice – Palliative Consulting Team.
Hours: Full-time - 40 hours 8A – 4:30P Monday - FridayShift: Day – no weekend, holiday or on-call Â

Responsibilities
- Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families.
- Perform a wide variety of duties including but not limited to bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc, and serving as a nursing resource for education and consultation to members of the interdisciplinary team.
Qualifications
- Requires a Masters of Science Degree in Nursing.
- Certification from a national accrediting body to practice as a Nurse Practitioner is required.Â
- Current RN license in the Commonwealth of Massachusetts & Rhode Island is required.
- Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.
- A minimum of one-year Nurse Practitioner experience.
- Certification in Hospice and Palliative Care required. Must possess current CPR certification
- Certification from a national accrediting body to practice as a Nurse Practitioner is required.
- Current RN license in the Commonwealth of Massachusetts is required.
- Certification in Hospice and Palliative Care required.
- Must possess current CPR certification.
Pay Range
