General Statement:

Ā 

Under the direction of the ACTP Supervisor; performs responsible tasks in providing comprehensive, intensive, off-site clinical services to Genesee Health System clients assigned to the ACT Program; responsible for significant liaison and consultation work with ancillary agencies, courts, hospitals, the police, and families, as well as direct intervention to clients assigned to the program. Ā Will not be assigned individual clients as clients will be assigned to the team as a whole, thus, a good deal of sharing of information, feedback, and teamwork is required; this is a 7-day-a-week, 24-hour coverage program and as such, persons will be required to have ā€œon-callā€ time where they may have to perform emergency interventions in person or otherwise, at odd hours; most client contacts will be off-site, either in clients’ 

home or mutually agreed upon sites; persons must be able to work effectively with minimal supervision; persons will need to facilitate the exchange of information between various programs, hospitals, and interested parties. Ā Performs related work as required.

Essential Physical Demands:

  • Vision – Position requires the ability to visually assess details at both near and far distances, as well as to perceive depth and spatial relationships with or without reasonable accommodations.
  • Hearing – Position requires the ability to receive and respond to verbal communication with or without reasonable accommodations.
  • Lifting, Standing, and Walking With or Without Reasonable Accommodations:
    • Position requires the ability to lift up to 35lbs on an occasional basis
    • Position requires the ability to regularly stand for extended periods of time
    • Position requires the ability to walk at a minimum of 35% up to a maximum of 65%
  • Position will require the ability to occasionally stoop, bend, or reach to perform work tasks with or without reasonable accommodation
  • Position must be able to perform tasks requiring manual dexterity, including the use of hands and fingers for handling, grasping, typing, writing, or operating equipment with or without reasonable accommodation.
