Responsibilities include: Evaluate patients in their homes and assess mobility, strength, balance, coordination, and functional limitations Create personalized therapy plans based on the patient's condition and goals Deliver therapeutic exercises and interventions to improve function and quality of life Provide gait, balance, transfer, mobility, and strengthening training Teach patients and caregivers strategies for safe movement and continued progress between visits Develop and instruct patients in appropriate home exercise programs Assess the need for assistive devices, adaptive equipment, and home safety modifications Track patient response to treatment and modify interventions as progress is made Maintain accurate and timely clinical documentation Identify and communicate changes in patient condition to the appropriate members of the care team Coordinate services with nurses, occupational therapists, speech therapists, social workers, and case managers Encourage patients to reach realistic functional goals while promoting independence and confidence Deliver care with professionalism, compassion, and respect for each patient's individual circumstances What We ValueProfessionalismWe take pride in providing dependable, high-quality care.
