The PRN Registered Nurse is an integral part of the hospice team and contributes to our team by delivering comprehensive hospice care when assigned. The PRN Registered Nurse will be working collaboratively with our hospice IDG and demonstrate nursing excellence as they care for the patients entrusted to our services. This role is instrumental, as the right candidate demonstrates exemplary patient care when providing necessary PRN visits.  The PRN Registered Nurse is part of our excellent hospice team. The care they provide demonstrates their commitment to their nursing profession and their impressive focus on quality and service excellence. The PRN Registered Nurse will be supported and equipped to bring out their nursing strengths as demonstrated by their ability to:

  • Deliver excellent, individualized patient care experiences driven by the patient care plan.
  • Treat patients and families as the unit of care, focusing on their feelings and holistic needs.
  • Manage illness progression and distressing symptoms with both pharmacological and non-pharmacological interventions.
  • Provide training and education to patients, families, and caregivers on symptom management, disease process, and advance care planning.
  • Collaborative, effective communication with the unit of care and the hospice interdisciplinary group (IDG) to meet the patient and family needs.
  • Effectively conduct goals of care conversations and manage patient/family expectations during planned visits.
  • Demonstrates culturally competent care and promotes patient/family satisfaction through excellent service and adherence to clinical policies.
  • Provision of care that naturally demonstrates exemplary customer service for patients, families, and referral sources.
  • Ensuring all clinical policies and procedures are followed to promote excellence in clinical practice.

Responsibilities:

  • Knowledge of federal, state, and local regulations r/t Hospice.
  • Knowledge of Medicare Conditions of Participation for Hospice.
  • Knowledge of quality standards and reporting practices for hospice and palliative care.
  • Knowledge of adult learning & training methodologies.
  • Knowledge of the philosophy of comfort, dignity, autonomy, quality of life, and empowerment.
  • Knowledge of common grief responses.
  • Knowledge of the Big 5 clinical expenses (labor, supplies, pharmacy, DME and mileage).
  • Practices within the scope of license, certification, or registration, where applicable, and as required by law and regulation.
  • Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing centers, assisted living communities, etc.
  • Practices safety and infection control methods.
  • Ability to challenge processes that are inefficient/ineffective while contributing to the solution.
  • Proficient in Microsoft Office (excel, word, outlook, power point) & Zoom / TEAMS.
  • Excellent problem-solving skills.
  • Strong organizational skills and ability prioritize tasks, detail oriented and flexible.
  • Excellent oral/written skills.
  • Excellent interpersonal skills.
  • Convey warmth, confidence, and competence.
  • Ability to work under pressure to meet deadlines.

Qualifications:

  • Registered Nurse.
  • 2+ years of nursing experience.
  • Prior home health or hospice experience is a plus.
  • Active Registered Nurse license in the state of SC.
  • Current CPR or willingness to obtain.
  • Valid driver’s license and proof of automobile insurance (or other mode of personal transportation).
